Qualifications: Master’s Degree in Public Health, Health Care Administration, Business, a health care field, or other related field required. Six (6) years of progressively more responsible experience in community services, public health or ambulatory care administration and must include experience in a management position. Demonstrated ability to manage multiple time sensitive projects from inception to completion. Experience working collaboratively and cooperatively with diverse groups in a medical practice or hospital preferred. Must possess excellent written and verbal communication skills and experience with data management and/or metrics. Proficiency with Microsoft Office Suite required. Candidates with a Bachelor’s Degree may substitute additional related professional experience for the Master’s Degree. Ideally candidate will also have some experience working in a clinical service (i.e. Nursing, Social Work, Care Management, Nutrition).
Responsibilities: Under the direction of the Chief Strategic Integration and Health Equity Officer, the Executive Director Community and Population Health Services, applies expertise in population health and population health management trends and best practices, models of service delivery and strategies to help achieve health equity across diverse patient populations through the planning, design and implementation of community and population health programs. Leverages social determinants of health (SDOH) framework to understand the role of upstream factors in the distribution of resources that impact life exposures and health outcomes. Works in collaboration with community, state, local and other constituency groups to execute a robust population health strategy. Manages program operations including the direct supervision of staff for all programs. Designs and constructs the quality monitoring and performance improvement tools necessary to ensure high reliability for all programs. Develops project plans using leading project management tools to achieve milestones and meet deadlines within budget. Demonstrates a strong background in population health management tools and data analytics in order to categorize populations by health risk or disease burden and displays the keen ability to translate data into action. Participates in all required committees, taskforces, team assignments, and collaborates with constituent and partner organizations to ensure the overall programmatic goals for the department are achieved in a timely fashion. Manages the interprofessional staff that are responsible for delivery of the Community and Population Health programs. Develops a culture of accountability in collaboration with colleagues, constituents, patients and clients.
